Yes - I can provide you with a Release of Information form that will allow me to contact another provider with your permission, and share/receive information that you have agreed upon.

Co-payments/Co-insurance and deductibles are due at the time services are rendered. Charges may be billed to your insurance company, however, any charges not covered by the insurance company are the responsibility of the client. If you decide to not go through your insurance, or I am Out-Of-Network, the entire cost is your responsibility.

If you do not show up for your session or do not cancel at least 24-hours prior, you will be responsible for a $100 charge. In instances of emergencies, please discuss this with me.
